The first three hospitals are just tutorial levels. After that, the game gains complexity.
It like old HOSPITAL (game from 90s) but with new graphic and high resolution textures. Better call it HOSPITAL REMASTERED
4th and 5th hospital is ez too (only earthquake can make some problems)
You can however try doing Sandbox with various handicaps in place like I do, like Junior Doctors only.
That's like with any game or quiz or skill. Once you learnt it, it become second nature.
There is definitely a learning curve to this game. I bet most of these posts here are just barely getting past one star and saying its too easy.
There getting through the game levels, and then there's making your hospital efficient and cost effective. The latter takes a little more time than the former to learn and know.
Overall, TPH isn't very hard but there are some missions like Duckworth-upon-Bilge that can put you out of your comfort zone. If you are great with management games and want a real challenge, TPH is definitely not the best game for that except if you play in sandbox mode and adjust sliders and settings (like the income multiplier) to make it more difficult for yourself. ^^
I'm yet to encounter anything in this game requiring the development of any type of skill.
