Can you actually lower your reputation?
I said this in another thread, but I could be running a haunted plague town with zombies up to their knees in♥♥♥♥♥♥and withered plants charging all of them +100% and still have max rep. A few times I've sent literally every single patient home angry, hundreds of them in one go, and not noticed an impactful change in reputation. I've intentionally let hospitals go to aboslute hell just to see what it would do, and these bastards just keep showing up and apparently having a great time.

It seems like the reputation system is meant to be part of a checks-and-balances system where bad treatment results in less patients showing up, giving you time to recover from being overwhelmed, but in practise it doesn't seem to work that way. What the hell am I missing?

i actually have zero reputation most of the time cuz i dont care about the patients as much as i should .. prices are maxed, patients keeps dying .. i never have problems with money, but i also dont have any rep .. there is no punishment for NOT having reputation so why care .. and if i want some, i just do campaign for reputation .. big for whole year, oh look, my rep is at max level .. but i dont actually see any difference between zero and max reputation ..
there should be some reward for yearly awards .. some motivation to actually do something .. there is no punishment for anything

agree please make the game more difficult! if somebody dies or pisst off it must hurt the hospital more than right now.maybe make a hard game option.so we can choose.

Yeah rep doesnt seem to work very well. only time my rep has ever gone down was when I jacked the prices to +50%

Only real incentive Ive seen for rep is as a star goal (never really compared patient #'s coming in at good and bad rep)

Another way to lower rep is to fail curing anybody during an emergency. So, take the emergency and close the room.

I had way too many patients piling up so I jacked up prices for a bit. Rep plummetted.

This is so weird. I've done all those things, failing emergencies, jacking up prices, sending everyone home, you name it. Perfect reputation. I'm starting to think this is a bug, but I'm not sure.

One problem is the good things give you twice the amount of rep than the bad things take away. One example is curing a person gives you two rep while someone dying takes away only one rep. I know that is not much but it all adds up.

You can easily get your rep down if you raise your prices to +100% from the start of a level. If you have a high cure rate (good machines, staff) though then it gets hard to tank it.
