You just have to impress the Yeti in one of his VIP trips.

Any of the three but its easier to do it on the first one.
Just make sure your hospital is clean. And all rooms prestige 5.

Toxic bins help too.

I somehow got it but only one room of mine was level 5.

No clue, just played the game normally and somehow I managed it.

You need to get him at his highest point, he can be 'happy' but its harder to make him 'amazed' by your hospital

It's a long time since I played but he needs to give you a reward to trigger the achievement.

In my last comment I was slightly wrong, you just need high prestige and a relatively clean hospital. He also like rummaging in bins...I had loads of bins so that might be what gave me the reward.

Try to follow his steps and what rooms he wants to visit - and then - before he comes inside - imrpoved them for 5-level (also replece poor plants and trashes).
Take care on his way between room too (try to control level of the plants, delete full trash cans, and check the attraction area (on attraction preview - just keep it green). You don't need to care about the rest of the rooms - in my case it worked.

I got this achievement in the first of the DLC hospitals. Honestly I don't know what impressed him, it was just the beginning of the level, my rooms were only level 3 and I didn't have anything special there. I was quite shocked when the achievement popped up.

When i get the review he is "so pleased" and i get reward. But no achievement. so do i need to do more ?

That's the second best result. In the best result, he will write that you "totally exceeded" his expectations.
