i've upgraded all my machines and have many similar type of treatment rooms, but my patients keep dying and my cure rate can't hit 90%...

Specialise staff in treatment relevant to that room, or the treatment general skill if there's no room related skill via training and promotions, then go to the staff job assignment tab of the staff list and restrict those rooms to the best staff only.

Upgrade the rooms with items that provide bonus treatment percentages like medicine cabinets. You can get about 30 medicine cabinets in a 4x4 treatment room in some cases, although clinics will not allow you to put them in so you'll have to rely on upgrades to the machine and staff.

It's most likely the staff. Check to see what doctors/nurses are doing which jobs! You might have a lot of staff, but some are better suited to actually healing people than others.

There's a hidden part to successfully treating a patient; they have to also pay for the service. lower your prices to achieve the end goal and it should work out.

Assigning staff to proper rooms, and inspect logs for patients that die. Make sure their stay isn't getting too long (less than 200 days until diagnosis is complete is generally my magic number).

Make sure your diagnosis rooms are clustered, so their not running across the hospital for a scan. Ensure you have specialized staff in both treatment AND diagnosis. Make sure you have good GPs. Getting 60% diagnosis on their first visit is wonderful.

Play with the queues and inspect low-health patients.

Keep in mind running to the toilet/cafeteria on the other side of the hospital causes serious delays as well.

Even still, your going to lose 97% patients often enough to notice. So sometimes you just have to wait and get lucky to nudge that 90%

Yes, every few months or so, or after a bout of training or recruitment just go down the doctors or nurses list and click on and off a few boxes.
