Playing online?

I just earned three stars for the first hospital but the online button in the top right is still greyed out (beside the ongoing challenges button). Does this become active later in the game or do I need friends playing online?

You can't on the first hospital. Only Hospital 2 onwards

As far as i can tell it's only steam friend leaderboards and not global. So add people if you want to get more people to challenge.

I think the Challenge mode should be a bit rebuild.

Maybe 3-5 Hospitals for different diificulties. And then a few challenges for each hospital.
a timelimit for each challenge and after all then a global and a friend leaderboard.
