In most London Train Stations and a lot of Shopping Centers (Malls) they don't have doors leading into the toilets, just an opening and a angled wall

Probably a couple of real life design reasons. If people don't wash their hands and touch the door to go out, the door has germs on the handle. If you've washed your hands they're now covered in germs from the door on your way out. No door = no opportunity for germs to be passed on.

And another one I suppose would be; how can I say, "smell ventilation".

This game is nonsenical af. The developers are probably trolling. Have you also noticed how they put windows on GPs offices? Privacy doesnt seem to be a concern.

Theres actually a simple Reason for that: In one of the earlier Releases (pretty sure it was beta) there was a Wall right after the open Door (ive seen similar Techniques in Buildings where i live). The Wall was removed quite some Time ago, but the open Doors stuck.
