All areas marked as cleared?

I just started my first playthrough on PC yesterday, and whenever i go to discover a GRE Quarantine or an Anomaly or even a train station, it says i've completed it with a checkmark or it's marked as green. However, i can't actually fast travel there, and there is still loot.

To add to this, i noticed above the ones i actually complete, it says ACTIVITY COMPLETED.
