I haven't played in a while, but have you tried swapping your difficulties around? You know going from hard to easy, and back to hard and seeing if that fixes it. Maybe fast travel between difficulty changes to ensure the game has some time to register the difficulty changes.

Sorry that I can't do anything better than guess, I hope you figure it out.
