"Wall of Valor" quest bug

After i delivered the final dogtag (Leon's dogtag), the Wall Of Valor: Lost Transmission (Brasco and Atom) subquest became again active and wants me to deliver their dogtags. I cannot deliver them because i can't speak to Cyra anymore, and the quest cannot be completed.

I have a weird bug in the final quest: The door to where Leon's body was last seen is closed/locked

But I have a similar bug with a PK mission "Deserter". The woman Im supposed to talk to has disappeared and the rest of that quest cant be completed.
