Entering Embodied Saint form

Is there anything special that I should be wearing and how do enter it on a keyboard and where is the faith icon located at..

You just need to have enough faith and hit 'X' on the keyboard. It worked with whatever I wore.

You just need a full faith bar (circle around your armour skill) and press X or L3 or R3 on controller

I think it's bugged. My faith stops climbing after I reach 100 faith. I'm unable to activate the ability.

Check in keybinds just in case you rebound the X button.
