Enchant, psalm and shard do stack once within the same item.
You can't have 2 psalms or 2 shards in the same item.
So, on my belt item I had a 20% enchant + 12% shard and +50% psalm and all worked
I tested on the last campaign boss with fear aura + vuln on debuff signum for gutanteed 10 stacks, with one enrage token and heat aura damage ticks.
I had a total of 128% vuln efficency on other gear.
only belt enchant: 2251 damage
belt enchant + shard: 2329 damage
belt enchant + psalm: 2575 damage
belt enchant + psalm + shard: 2653 damage
But that's really strange, what and how doesn't it stack then.
Though so. Seems like bad description though. More like doesn't stack from the same item type or source in one item.
