When offline, it can't retrieve your old ID, so it assigns you a new one and drops you into the world as a clone.
The other you in the world still has the items you had when you logged off, so the only way to get that stuff back is to kill your clone.

The case you describe is yes a known issue, and not alien to the devs if the game can't connect to your Steam ID, for some reason (either Steam is not running or it's loosing connection with Steam when saving/reloading a save (effectively offline)).
So when you loggin you might discover that there is a clone there where you last was before quitting the game (or the game crashed).
What do if your old self is in a vehicle? Is there any way to move/destroy the vehicle or driver?
To prevent this from happening you need to check several things:
1.) Make sure steam is online and logged in before starting the game.
2.) Make sure your internet connection is working before starting the game.
3.) If you see "Unable to establish online session" message when you try to load your save then STOP AND DO NOT LOAD YOUR SAVE. Exit the game entirely back to windows desktop and try to start the game again, then try to load your save again.
If you are still seeing the "Unable to establish online session" message then completely exit the game to the desktop again, check #1, #2, and possibly completely close and exit the steam client, re-open the steam client then try to load the game again.
I would expect explosives to damage characters no matter what they're doing if in range, but this does not happen, having it do so seems to be the easiest way to fix it.
