Technically you can't know even in multiplayer.
In multiplayer you see other player, not yourself.
Only way is to ask them to tell you what they see... and believe (or not believe) them.

If it continues to be asked then it continues to be a concern. While I am not daunted by playing as a female, it is unnatural for me, because I am a male. If a female told me she wanted the ability to play a male in a game, such as this, where the character is not actually a persona with defined characteristics, I would not feel she was being sexist, nor would I accuse her of such things. This is because I would not be offended.
But I didn't make that ask, I just asked to see if this was the case and you have confirmed for me that is a virtue signal, and as such, I shall ignore it entirely while continuing to enjoy the game. Thanks for playing, stay offended.
