I notice you're using two monitors that seem to be mapped to a single desktop, this is the only thing that looks even slightly unusual in your system so let's get some of the basics here out of the way: ensure your monitors are plugged into the graphics card and not the motherboard, if they both are then try disabling all but your one primary monitor and running the game again.
If there's no success there the next thing to try would be using the NVIDIA control panel to force the game to use your graphics card. I'm not at home right now so I can't look up the exact buttons and tabs, but in the control panel you can go to a per-application list (add the Neir executable if ncessary, though it should already be on there). There should be an option there saying something like "Display Adapter: Use System Default"; change this to force it to run through your graphics card. If that doesn't work it wouldn't hurt to go through that list again and force off all the advanced settings (antialiasing, etc) to try to get the game using as little graphics memory as possible and trying to run it again.
The only other thing that I forgot to mention (although idk if it means anything at all...) is that I'm curious if it's something wrong with my directx 11. I see that this game uses it, and I also get a directx fatal error when trying to run FFXIV on directx 11 (but not on 9). For that game I can get around the issue by just running on dx9, but I was wondering if there might be some kind of link...
I did run the dxdiag tool and it didn't find anything odd so I kinda ruled it out.
