So about the second playthrough (possible spoilers)
I have heard that the fourth playthrough is a continuation of the story, is it a continuation of my first playthrough or my second playthrough. Like if I do all of the side quests in the first playthrough but not the second will they still come up in the fourth playthrough.

There's no fourth playthrough. Only A, B and C (D is just an alternate ending)

I'm assuming that what you're asking is if playing after you complete ending B, how things will proceed? If so, then the answer is that it is a continuation of your first and second playthroughs. As for side quests, once you complete a side quest, it is forever completed on your save game and will remain so no matter which playthrough you're on...assuming you've completed said side quests in full. So no, if you complete the side quests in your first playthrough, those completed side quests will not appear in the fourth, because honestly they wouldn't even appear in the second one. ;)

Goes like this:

1st playthrough (A) > 2nd playthrough (B) > New content > Ending C or D > Chapter Select > Whatever ending you didn't get at the path of C or D > Ending E

You cannot miss any side quests, use chapter select to go back to them to finish. Only Ending Y is missable if you don't let Emil clones self destruct, then save after killing them. Have to restart the entire game to get that quest again.

Only the Resistance merchant quests need to be repeated on the second play. Those look like side quests but are simply part of the main quest, as routes A and B are the same timeline from different perspectives. Any sub quest that you started on route A, but didn't finish, you will have them available on either on route B or on subsequent playthroughs by using chapter select. Some quests will give you the idea that they were finished because you get no new pointers, but sometimes they are not in reality finished. For a side quest to be fully finished, you must get the "[sidequest name] completed" prompt on the middle of the screen.
