Yeah, it's not just the phone - depending on the timing duration of the button you use to toggle between the HunterMate and the current inventory item, the swap may fail (as in your case), or both disappear and you are left with nothing in your hand (which is not necessarily a bug, I think).
If you toggle again, it should reset. Or, if you are trying to get back to your current inventory item, use the wheel and select it again.
Yea that happened to alot of people because of the new Loadout system I hate it honestly, way more clicks and switching around then it was before
