First off, this list is by no means complete and/or totally accurate. It's based on my own (and other players, should they care to contribute) observations. Feel free to make corrections or help fill in the blanks and I'll update this with the new info. =)

I spent hours trying to catch these leaves as they fall. They're usually too quick for me.

I'm gonna draw the biggest circle! And I'll add some fancy designs inside just like dad does! Whoa, why is it glowing?
Significantly increases the size of magic circles created when casting spells.

Copper Rod

If I'm to enter the storm with this, I should at least take some precautions.
+Lightning resistance. Stacks with itself, but has diminishing returns.

Cracked Mirror

I hate you even more than I hate myself. Try me.
Sometimes makes you go berserk when hit. Attack faster, and (?)do more damage for a brief time. (Nerava)

They said I could store a spell in one of these. Maybe I won't have to wait so long this time.
Sometimes(?) it completely resets the spell cooldown allowing you to cast two consecutive spells. (Darth Grin)

Dancing Shoes
50 Exp

Gives a damage boost when you successfully dodge an attack (traps seem to count too). Indicated by white smoke effect around you. (Nerava)

Dinner Plate

I don't regret the extra helping.
Increased hunger recovery from food items(?)

Doorknob
Exp
Dammit! This stupid piece of metal shocks me every time.
Casts a weak spark spell when you pick up metal items (metal, keys, armor, etc.)

Dueling Hilt

The riposte, if performed correctly, will be vastly more effective than brute force.
Gives a damage boost when you successfully parry an attack. Indicated by red effect around weapon. (Nerava)

Dying Candle

There was a sudden warmth between my shoulders. The last thing I saw was the candle my daughter created.

Summons a ghostly archer to help you out. Archer can die and doesn't come back. =(

Can't forget this! I'll stop every few miles and patch myself up.
Triggers when entering a new level, probably heals. Amount unknown (I was at max anyways). (Sheesh)

Half a Shield

Why are my trousers way over there and whose legs are in them? Where did the other half of my shield go?
Improves cleave stat. “All weapons have a hidden cleave stat that adjusts the damage and power falloff after each enemy hit. For example, blunt weapons deal significantly less damage after passing through the first enemy while axes can effectively cleave through multiple enemies. Half a Shield improves the cleave stat and can even cause some attacks to increase in damage and power after hitting an enemy.” (Julian Edison)

Headsman's Axe

Occasionally insta-kills enemies(?) or at least dramatically increases damage of a hit.

Head of a Statue
Exp
He rent the ground with such ferocity that even nearby statues untouched by his hammer toppled to the ground.

Heavy Ball

I hate this game! I keep getting hit in the face.
Materializes a ball that circles around your character, it hits objects and creatures and sometimes stuns the smaller ones (like rats or small blood puddings). (Darth Grin)

Sometimes you hide when enemies are aggro towards you (character fades a little) and they have to reacquire you as threat (will have the question mark pop up again when you move and they spot you). (Sheesh)

Softens the blow from bludgeoning attacks.
+Blunt resistance. Stacks with itself, but has diminishing returns.

Leech

There is more blood here than could ever need.

Occasionally deals fire damage to an enemy when attacking.

Occasionally find random items in crates and barrels.

Well don't just stand there, man! Break it open and let us see what mysteries lie within.
Will drop 1 exp gem when breaking crates, barrels (and statues ?!). (Sheesh)

My Best Friend's Grave
Exp
The memory of his death is still painful. Physically painful.
Triggered on killing enemies. It hurts other enemies for just a few HP that are close by to the dying one. Has a nice chaining effect. (Sheesh)

Nail of a Giant
Exp

Makes your weapons bigger.

Needle

I do not need to rest. In fact, I may never rest again.

Why even get in the water without one?
Increases speed in water. (The Potatolord and Sheesh)

Ordinary Cloak

Just an ordinary guy in an ordinary cloak. No need to pay me any mind.
Decreases the distance at which enemies aggro(?)

Pickaxe
Exp
Break the stone, smelt the ore. Break the stone, smelt the ore.
Occasionally causes stones, pots, and statues to drop a piece of metal as well when you smash them.

Piece of Nothing

Pile of Ash
50 Exp
The corpses... exploded. Naught was left but ash.
Occasionally causes corpses to explode.

Plague Tick
Exp
The sickness spread everlasting, eventually toppling an empire.
Applies weak poison damage over time effect to enemies that you touch. The effect can spread from enemies to other enemies at that point.

Poisoned Apple
25 Exp
The coward's weapon comes in many forms.
+Poison resistance. Stacks with itself, but has diminishing returns.

Propeller
25 Exp
Almost finished! Only need to find a little more metal.
Casts Levitate on you when you pick up anything metal

(The) Quill

The scholar's weapon.

Rallying Horn

For the king! For glory and honor!
Increases your surge count by one (restores 1 surge too) and increases the size.

Sacred Breastplate

It truly is an angel, adorned in blessed metals that mend any scratch. Sincerely beautiful. I will not survive this.
Tricky one, It might have caused an effect when I got damaged, but I didn't see its icon pop up. I got damaged, then fully healed and also got a comfortably large armour bar on top of that. I'd expect that to proc on a very low percentage. (Sheesh)

Shard of Glass

The reflectiveness of magic is highly exaggerated.
+Magic resistance. Stacks with itself but has diminishing returns.

Shiny Coin

I should be dead... The arrow was deflected by my lucky coin.
Sometimes negates hits. It seems to have a higher chance than the Tortoise Shell, but the chance doesn't increase as you get hit. (Nerava)

Spoiled Meat
25 Exp
This crate is completely spoiled, only flies in here. Disgusting.
Summons insects that follow you when you smash open barrels, crates, and pots. They attack enemies and then die. Needed for Lord of the Flies achievement.

Stepping Stone
25 Exp
I spent hours in the garden practicing my footwork.

Some people call it cheating. Those people missed their own deaths.

Awesome, Rhawkas. This is great :D

I'll give you one: Half a Shield. All weapons have a hidden cleave stat that adjusts the damage and power falloff after each enemy hit. For example, blunt weapons deal significantly less damage after passing through the first enemy while axes can effectively cleave through multiple enemies. Half a Shield improves the cleave stat and can even cause some attacks to increase in damage and power after hitting an enemy. If you're crafty you can utilize that to break through rocks using a weapon that would otherwise be too weak.

You missed stomping boots. They push all enemies away from you when hit. Has a cooldown.

Suppose I'll toss a pair your way, since they're quite similar.
Duelling hilt gives a damage boost when you successfully parry an attack.
Dancing shoes give that same boost when you successfully dodge an attack.

Probably give some more info later, don't really want to flood this comment. (Also, I'm kinda fond of blind playthroughs.)

Although I will semi-cryptically hint to watch your footing.

Yeah, there are a lot of items to find (much less figure out what they do XD ), so I'm sure a bunch are still missing. XD Thanks for contributing. =)

How about...

The Bloodshot Eye as well, but I have no clue what it does!

Ghost Blade: A second weapon that is swung slightly after the first
(two attacks for the price of one) (it is shown as a semi-transparent second weapon in your hand)
(not sure if it has a chance to be any weapon but my mh is flamberge and oh is bow while gb is a cleaver)
Bloody Shield: (procs when taking damage and seems to nullify that damage tick)
Sacred Breastplate: ???
Bone Saw: ???
Notice of Bank Error: (procs when making an xp purchase - seems to refund/nullify the cost)
Frozen Chain: 50xp, ???


Sub-Equipment
Frozen Crown: Leaves a trail of ice on the ground that chills/slows enemies.
Belt of Strength: ??? (it didn't increase my Str% stat)(maybe knocks enemies back more?)

Pretty sure those three are
Sacred Breastplate allows overhealing into armor,
Bone saw is higher crit chance at lower health,
Belt of strength allows you to hold attacks, similar to bows.

Doesn't the notice of bank error make you get the same experience instead of paying it, not just nullify it? (very rarely though)

Golden Lock: Chests (wood/steel) no longer contain any items whatsoever but instead contain 1 mouse/rat.

Is this a bug?

[EDIT: after 4/5 chests with only 1 rat each the next one had 6 keys, 2 healing meat and 1 surge]
[Unfortunately 3 of the ones containing nothing were golden chests so I got ripped there]
