Low FPS and GPU usage with RTX 2080TI on 1440p Max Settings
I only get around 40-60 FPS and 50% GPU usage.

it has neither to do with Nvidia/Windows Power saving settings, a CPU (i7 6800k) Bottleneck or G/V-Sync.
I have the latest Nvidia Drivers and use an SSD.

I know that the game is not really much optimized but the low GPU usage seems really low in comparison with what i saw on YouTube.

Does anyone know a fix or experience something similar.

I know it's a pain to comment on something so old but I'm having the same problem.

Same resolution with 2070 super. average 50-70 frames but it doesn't look smooth when its a 70. Changing graphics from max to min barely affects the fps as well.

My gpu and cpu aren't being fully utilised. Maybe this is a problem for RTX cards?

I haven't found any fix. Hopefully they will patch it when they release the other characters.

I was having terrible performance issues as well, but gained massive improvements by applying a fix that worked for another Unity based game, Escape from Tarkov.

Reddit thread about the issue.

By modifying the file: Pathologic\Pathologic_Data\boot.config
And making sure the following values are set:

gfx-enable-gfx-jobs=1 gfx-enable-native-gfx-jobs=1

Thanks for this, went from 50% GPU usage and 50-55 FPS to 95-98% and 80+ FPS on Ryzen 7 2700, GTX 1070, 16Gb 3200, SSD.

Why do I only have gfx-enable-native-gfx-jobs
and not the other?

I added the other line in and I did notice a difference. I can now max everything on the sliders at 1440p and get average 90 fps.

Edit : not 90... 165 or locked at a solid 120.

Before that I was getting micro stutters and not a smooth frame rate while maxed so I had a few things turned down. It is def smoother now even with the sliders maxed.

Using a 3080 gpu with an 19 10850k.

Also my bad im getting 165 frames ( not 90 ) but I lock it at 120 so I can use 10 bit color on my monitor.

How can i access boot.config file? I've found it but can't edit it

Edit* Nvm I found out how to do it and tested the results. The game runs great now!

It wasn't an option. I downloaded two programs, first one didn't let me edit it but the second one did

You can open .config files with Notepad, or Notepad++. Any text editor will work.

What you need to do is right click the file in your Windows File Explorer, and say "Open With..." and then choose Notepad or Notepad++.
