It's not necessary to play the first to enjoy the second, but it does have some elements that mean old-time players will have a slightly easier time.
So is more like a sequel? I mean the plot is different or is more a "nostalgic thing" made for the ones who played the first Pathologic and want to play it again but with bette graphic?
You can quite reasonably just play the new one. If you don't want to wait for the other two playable characters though then you may want to consider registry tweaking the game and playing Clara's route.
If you'd like to get more story details from the original game then again, I'd suggest tweaking the registry to unlock Clara from the start and plaything the original game with her. A lot of what you'll have learnt playing this new game will still be useful.
The alternative is to wait for the other two characters in this new game. I'd honestly just recommend getting stuck into this new game as it stands on its own merits - even without the other two playable characters.
And well, there are plenty new things in the story of Pathologic 2. So, if you find yourself liking any of these games, it's highly likely that you end up playing both of them.
So, don't worry which one to start, just enjoy the story
If you were to just play the same character in both games, you'd still find quite large differences in the story let alone the mechanics. I'd encourage you to play the new game because it is mechanically stronger - a story can be looked up for a plot summary, and no matter how many people gush over Pathologic's story, it holds true - the story can be looked up and read, the mechanics need to be experienced first hand.
