Beating "Director's Cut" in "Typing of the Dead" has like 1,2% completion rate and that isn't hard at all, but I guess people haven't felt like playing it so much...
Some of my thoughts while playing at that difficulty:
Sometimes the turret sections can be tricky as you can't move to cover and die in a few hits.
The stealing water level is easily beat by just constantly reloading and firing without pause.
Sometimes you find it challenging to protect the AI squad while not dying yourself, e.g. they both are downed and you have limited time to clear the area and save them before they bleed out.
One shot weapons are pretty good friends to have, pistols are good for this such as Desert Eagle and Magnum.
Those tend to help if you genuinly struggle with it, although most times when you die again and again on one area this can be solved by taking a break to refresh your mind. Hope this helps, good luck.
It's just not fun. Even in the parts where you don't need a lot of luck to have any chance of suviving and have to reload over and over it's duck out of cover, get off one burst shot, get wounded to near death immediately, duck back in cover, sit there doing nothing for 20 seconds waiting to heal, repeat, repeat. Gets old fast.
In short, almost every cover becomes a deathtrap, 'cause you have a limited angle of aiming and movement and you die a lot easier. And I mean two bullets and you're done. So yeah, popping out of cover to shoot dudes is pretty much useless after first two levels. Instead, you'll have to utilise free movement and crouching. Of course, cover remains very important, but it's best to avoid gluing your back to it and instead just crouch behind obstacles and switch camera angles between left and right shoulder. It's also best to wait until enemy comes to you. They won't be respawning indefinitely, so there's no point in rushing into crossfire. Find a good spot, dig in, and wait for them to come to you (which they usually do). Headshots are also important. Two bullets from almost any gun will do the job even on this difficulty level. Try your best to pop the melon in short bursts (gotta remember that ammo is hard to come by).
Also, disappointingly, your squadmates turn out to be pretty useless on FUBAR. All they are good for is sniping guys from a distance or silent takedowns, because the rest of the time they just block your shots or constantly get incapacitated. And it probably goes without saying, but having to babysit some hapless idiots while bullets are flying thick and timer is ticking fast, is fuc*ing bullsh!t.
Played FUBAR for half an hour and currently in chapter 3 without that much hassle.
Dun know how I'm gonna survive the Pit and the damn helicopter tho.

It also helps to know what's ahead and where to position yourself for maximum efficiency, because otherwise you'll either run out of ammo, or be stuck with weapons that aren't best for the next level.
I'm 58 though, and I've beaten this and Kane & Lynch 2 on the hardest modes several times each. So obviously it can be done without the reflexes and eyesight of a teen if you use good strategy.
Both those games offer a lot of challenge on hardest modes, but also become much more doable once you know the best weapons and tactics to use. Both of them are also similar in that they offer ways to use fast advancement. It's when you try to hold back and go slow that they become harder.
