update 25 they implemented an unfinished upgrade to melee by accident when they hotfixed another bug. Update 26 finished the melee improvement
Anywho though, yes I have definetly had the game not hit zombies right in front of my character. Mostly when using a bladed weapon and theres something else within the swing arc. Like furniture objects or even a bunch of dead zombies all around. So if like 4 or 5 pile up i move away a few steps.
Remember bladed weapons can only hit one zombie at a time, while blunt ones can hit multiple in the same swing.
havnt encountered that problem in 26, havnt tested 26.1 till now (when i do i hope the patch 26 NPC TP Bug is fixed, or the game wanders to hibernation again until that is fixed)
This has always been the case and is clearly a physics engine problem.
I therefore opt to fight in more open terrain and usually sprint-dodge in and out of all rooms in the house before running outside and waiting for them to get out or get in my car to smash them with it.
Edit: if this is not what you're talking about I haven't seen it. In that case, it is probably yet another "omen" that your game is in the process of glitching out, possibly in much more serious ways than just some zombie ghosting. My advice is to reboot the game if this happens.
