Even when there is a siege, the pacing between the "waves" are so slow that you basically get like 10 zombies at once and then need to wait a couple of minutes before 10 more arrive.
If anything, the sieges are even more of a joke now than before the rework of the system.
Edit; oh, and by the way - the noise from my base was at 55+ (the meter goes to 30)
Consider occupying good defensively placed outposts around your base and activating incendiary mines if you need time for your base to be at peace. Alternatively if you're struggling, try lowering your difficulty a setting or two.
With the (re)introduction of incind mines at outposts, unlucky jug/feral spawns become irrelevant. Also, since outposts destroy infesting/siege hordes, they are extremely useful in holding back the tide and preventing sieges. It is because of outposts, that I am on my third lethal community right now.
The new Plague Heart mechanics are pretty cool, but they make playing the game also a lot simpler.
Disturbing one can lead to problems, but they will develop slowly.
Disturbing two will most definitely be a problem (especially if they are far apart), but still will be negligible with good outpost placement.
Disturbing three or more and you should be solely focused on removing the disturbed hearts.
Destroying multiple Plague Hearts is actually not to hard, granted you have Scent Block (can be obtained for "free" via missions), a vehicle, a heavy (preferably not broken) weapon and cure.
Pop that scent and knock out those disturbed hearts, when they are destroyed, the infested buildings go with them. Be aware tho, each heart has its own unique infested homes, so if they don't start withering after you kill a heart, you didn't kill the correct disturbed heart.
You have to be extremely careful in the beginning when you start a fresh community with no legacy survivors. Killing Plague zeds in plague territory has a chance to awaken hearts, you either have to use a crossbow or silently take down zeds in order to not disturb the heart. Unless you have some really good traits, actively fighting an uphill battle with weaker characters is not such a good idea.
Should you get noticed by a pack of plague zeke (be they special or not), you should lead them out of plague territory before killing them. Better yet, lead them to an outpost so they can be taken out by the "newly" featured incind mines.

I whole-heartedly endorse trying this out in lethal zone, it took one or two runs to get the gist, but afterwards its smooth sailing.. until you fight hostile survivors, my only tip with that is high-caliber weapons and plague jars, lots and lots of plague jars.
As other said infestation got freaks depending on level, but to clear them you really only have to kill the screamers. You dont have to kill any others. But their loot is pretty good (OP in some ways i think)
Over all i think the game stayed effectively the same in lethal mid game and got significantly easier begining game. With the traps you wont have any trouble finding plague samples in the first 2 hours of gameplay. That first 10 plague samples is always the hardest
