cleared whole map from plague hearts but some areas still red?
what this red is supposed to mean? what is this zone?

Probably a Heart you haven't scouted yet?

Might have been a spawned one, from a mission or something. Have you tried the "locate Plague heart" radio call? Used a Survey Car?

The area that a plague heart was will be sort of "pink" on the map. Never goes away. Though it doesn't mean anything either...at least as far as I've noticed. You can make outposts there and the area doesn't seem to be any more or less dangerous than anywhere else.

I thought this shade of red was meant to indicate higher zombie population. IIRC these red colors existed before plague hearts were added. The hearts create the animated red fog effect.

all plagues have been cleared and i can finish legacy. but still red areas on the map (not "claimed red houses etc", just areas).

oh, so it was like this before plague hearts? i dont understand what it indicates, or how it calculates zombie density of that red area.

imho it indicates where plague hearts existing but it's not going away after clearing

I think its the zombie population. Red area = more zombies. I can't confirm it 100%, but from my experience that seems to be the case. Red area are usually in towns/less remote area and I also tend to notice more zombies on those area.

Actually this was in a change note a while back.

"We now shade the map darker red in places with dense zombie populations, and lighter grey in places where the zombies have been cleared out.

As you defeat zombies and clear infestations, you can actually see the impact your actions have on the map, and then watch the zombies slowly trickle back in after you leave.

We now also save the effects of your actions on zombie density, so when you come back to the game after a break, the swath you cut through the map is still waiting there for you to admire."

However, I have gone on sprees where I have killed hundreds of zeds...like just spent hours doing it...it can be great stress relief...but I haven't noticed a change. Maybe I just haven't noticed. To me it seems like they just respawn in about a minute or so.

This is the correct answer. You will notice these pink areas are in the town areas to indicate a higher concentration of zombies there.
