I dont notice the game getting harder, the plague hearts might be tougher to kill, but nothing dramatic changes that I notice. Also by the time your clearing out the last heart, your so strong and well equipped hat it doesn't matter.

was that long ago?... i mean i remember them saying last plague hearts becoming tougher but i dont even feel them or maybe barely .. i usually kill them with molotovs 4-6 before.
destroying all hearts will remove bloodplague zed and jugs from spawning making the game easier.
that was before and i don't have a clue about it now.

(note that i have not tried destroying any plague hearts recently.... cause i don't want to make my game easier anymore)

The density of zombies around the plague heart increases slightly, the game as a whole remains mostly unchanged

Does destroying all of them actually stop plague zombies from spawning? On my first playthrough I still encountered plague zombies but maybe they were just remnants from before I destroyed the plaguehearts and never bothered eradicating the last of the roaming hordes that had spawned?

I personally think it does. I play on Nightmare Mode and have done for some time now and I notice that when I get to the last few plague hearts, the hordes of zombies and freaks are endless. On Nightmare mode you literally have to take the plague heart down in under 10 seconds if you want to survive xD I don't think you'd notice much of a difference on Standard zone though as there's not as many plague hearts.

Yes, you will still get freaks and normal zombies, but all plague zombies will be wiped out.

Plague zombies will stop spawning but the ones that were already in the game will still be there. Therefore, for a certain time after destroying the last plague heart, you will still encounter them.

That's really cool from a mechanical standpoint since killing plaguehearts kills all the plague zombies around it I figured it meant all the plague zombies on the map would be gone the moment you killed all of them.

I actually prefer that the existing wandering hordes of them stay until eradicated. That's pretty cool.

I play on nightmare, and even killing the last heart on a map, I dont notice a big difference, though I do kill it within 10-15 seconds, so maybe thats what is helping.

Yeah, try not killing it so quickly and you'll see what I mean xD
