Dog Tag 27...dissappeared

Hi guys. Need a little help here. In my collectibles section it is saying I have absolutely everything in the one run through I did. I know I got everything! I was playing co-op with a mate aswell for most of it. However after all that time its saying that I am missing Dog Tag #27 in mission9....I have even used guides this time and I have all of them in this map...so what the hell is going on.

I cant complete the collection because the tag no longer appears and yet I dont have it in the stats but I know I picked it up!!....argh!

I appreciate the help, but what I mentioned was that I played through once with a friend & I got everything. Then I checked my stats and there is #27 dog tag missing....However, I go to where tag 27 is supposed to be according to all guides and as anticipated (especially as I remember getting it) it wasn't there!

So I have got the tag previously, has not shown up on my stats. Gone back to double check and is no longer there so I cannot get it? anybody heard anything like it and have any idea?
