Getting 5 stars should be a little easier?
I loved Theme Hospital, so i loved Two Point Hospital, and so far i also really like this game.

Dunno if its only me, but i think to archieve 5 stars is a bit too hard? It seems so damn hard it isn't even fun anymore!

I am currently on level 1 and i don't wanna continue until i got the dang 5 stars. I have been trying for hours now, i did hit 4,9 but 5 stars seems nearly impossible. Two Point was a walk in the park compared to this.

Should i restart the level? Because i can't seem to hit 5 stars and the ever increasing stream of patients isn't really helping either... my star rating even dropped again.

The difficulty of obtaining a 5 star rating in this game is a bit too much if you ask me. It's a tycoon and should be fun right? No souls game ;)

The first hospital is a bit too difficult for no reason in all honesty.

I highly recommend to play the story until you unlock teleporters and the second consultant (they are unlocked at the same time, and I believe it's chapter 3 from memory). Both of them make the game a LOT easier and make 5 stars in every level a much less daunting task.
I got five stars in all the story missions, but the optional hospitals I am stuck on one. As people have said, the issue is Patient Mood. This needs to be 86% to trigger five stars (85% is still orange). This is basically impossible to get once your hospital is established because you have such a high patient rate that you cannot keep them happy due to Congestion modifiers. Remember, some patients, even with all their desire boosts and no Congestion WILL still be on 80% and will never ever go higher. I even made every single doctor in my hospital a Dyoni for the Pheromone Mood boost and it doesn't trigger enough to keep everybody happy. My next attempt will be hiring doctors that 'make people happy near by' and hope that works.

So, all these people going 'It's not hard', yes it is due to how patients are coded and how the game loop occurs. There is very little you can do to stop Congestion (a single person next to a KB will make them have an orange debuff and two is red), yes, even with teleporters galore you will still get it and that will keep people down ON TOP of the default 'never goes happier than 80%' patients. Once again, I have done all of the story missions to 5* and currently stuck on one optional hospital (fourth one) with the other three also 5*. Again, it is down to timing and getting 5* in middle game, rather than end game, as end will not do it.

tl;dr it is bugged imo, patients will be stuck at 80% mood even with all positive mood modifiers and don't seem to get affected by doctor buffs. There are several other bugs such as patients having 'Messy Area' debuff even though it is clean. Even doctors get this and it is impossible to get rid of, so you have to fire them. They will likely get it again, regardless. Or Consultant skills not working. Or artefacts not seeming to work. Or loads of things. It's buggy.

Edit: I managed to get the 5 Star Rating despite not actually getting it on the screen. I assume this is a display bug as I have in several missions gotten 5 Star Rating on the objective screen display but it isn't and I guess I got 4.9 on objective screen display but didn't.... Or it is another bug and I shouldn't have gotten it. Not going to complain though.

Edit (again): It did eventually say so, the trick worked btw. I just hired Charismastic doctors to wander the halls without any rooms to man, combined with phereomones they give a decent boost (but the 'forever 80%' patients often don't improve or if they do start to go back down to 80%).

Since having all Drive-throughs opened up causes too much patient density to manage for 5 star mood rating, all the 5 star ratings I did get for the non-story Hospital locations (specifically the Xil and Cabal non-story locations) were done by me refusing to open up any room that was connected to an additional Drive-through; essentially staying with only one drive-through opened. As doing so allowed me to keep patient intake from the drop-offs low enough that it was manageable for the rooms I did unlock. Though that did require me to maximize my space usage for the rooms, and replace treatment rooms in order to fulfill objectives.
I hope the patch comes soon. Even if it's still possible to reach 5 stars, it is just pointlessly long and tedious now. Just not fun.

If you do this with spacious rooms, wide corridors lined with decorations and keep your queue times down to a minimum it's pretty easy to 5* stuff... So far anyway.

I had to restart the sandbox on the first one to get that to 5* purely because I couldn't be bothered rearranging everything after the learning curve. The two story missions after that and the side sandbox mission I got 5* without trying.

I was struggling to get to 85% on the patient mood before but now I'm over 90% so that extra boost from charisma isn't essential but it sure makes things run a lot smoother.

yeah just restarted a fresh profile on lv 1 again after finishing game.. and noticed that no matter what I do I cant make it jump past 4.9, without telepads and loamgates and no consultants on lv1 there is no way to "speed" it up or get more vict.. I mean patients to enter, so ive maxed the current flow of lv 1 with everything decked out cultre wise with lv1 stuff the visitor mood just hovers shy of optimal .. but im glad its not me

Once you've completed the storyline you stop receiving scrap, but the treatment reward penalty is still applied, so there's basically no way to pull your finances out of the red.
Yeah, maybe a small tweak would be good. On release I was able to get the first hospital to 5 stars with work. I'm on chapter 5 now and everything is stuck at 4.9.
I got 5 stars on the whole campaign and the non campaign missions. The first few were the hardest. The key things in my case were throughput, finances and plenty of diagnosis rooms. Apart from that spamming toilets, trashcans, vending machines, and lots of decorations, various kinds of them. Check your staff's salaries too, although later I didn't bother. Good luck.

Ps. Oh, and later don't build too many rooms. I almost ran out of space only in one mission and all my rooms have been spacious throughout the campaign.

Cheers. This is sorely needed, it seems near impossible to get five stars on the first level (and likely others) because of the Patient Mood requirement which is, to a distressing extent, kept entirely beyond our control due to the gantry issue.

I got five on the second one naturally enough while just playing the game as best I could; seems difficulty in getting to five stars is currently very dependant on layout, grateful it's being addressed.

Chapter 8 does seem to be weird. After you complete the story the extra cash stops coming in. In my game Baz has also stopped dropping by and the very low payment you get on that level combined with the game not counting the flying golden things that give 3,5 grand as income means that it seems impossible to get above 4.8 stars or something like that.

The progress counter on the right hand side of the screen also doesn't show the mission to improve the hospital to five stars. But the stars counter on the lower left side of the screen does working.
