Grass/flora mods help in general like you said, but no mod involves hand-placed plants with hiding seams in mind. Editing the landscape would be a big step forward in modding that I'd love to take. Exciting! I've only ever ♥♥♥♥♥♥ around making relatively basic mods in xEdit so it'll be slow but hey, that's modding. I can start with a rock outside Dawnstar's entrance, pick a winter-floral (after getting permission, of course), and very slowly start there with one edit. Wish my sorry ass good luck!
As to the actual doing, some things can be done in xEdit (which was how I applied my fix), and others will need to be done in the CK. Either way, best to start with an area that you are frequently in and which has the most annoyances, and fix the major ones first, then you can move to other areas. After several decades, you should have covered most of the issues - just in time for the release of TES VI.
