Unless someone confirms otherwise, the percent chance listed above is probably correct, but may take you much much much more than 500-600 kills with cultists to get it.
I remember seeing a (very) old thread that discussed transformation in more detail but it is probably over a year old.
Researching boons and using Chaos Lord reduce your transformation chance.
Tried "save scumming" for two hours (save just before 3 groups of cultists get the killing blow on three separate units, attack with all three, reload if they don't transform) with no new transformations.
Apocolocyntosis is probably the most time I've ever invested in unlocking a single achievement.
Edit: Steam is listening. 8 minutes after posting this, I got both transformations. Thank Christ.
