As for religion and gender stuff, it's mainly referencing PC Principle and his run in the series, so you can make cisgender or transgender etc. etc., and a decent selection of religion options too, but they pretty much just add to your "character sheet" and don't really affect anything either as far as I know (beaten the game multiple times, including Danger Deck Ultra). But you can look up other character sheets and see how they "self identify" which sets up show references too, like how Tweek and Craig are homosexual and stuff.
EDIT ::: I actually just learned something I did not know, about the skin color options. Apparently if you make your character black, Morgan Freeman will not fight you lol, kind of funny.
There is a character you meet that lets you change your look though, including skin color, so no worries.
Cops also don't like you if you are black.
And when cops are on the streets will auto attack you if you are black and won't if you are white.
