It's important to note that these events occur in a fictional universe, within the South Park fictional universe, where the kids and some of the grownups take part in a role playing game. By over immersing themselves in the story, Mitch was able to take on a more solid form and things got more real. Although originating from Cartman's severe psychological distortion, everyone's minds are so heavily engaged in the augmented reality of SOT and TFBW that the Mitch Conner entity spreads to other people's consciousness as a manifestation of people's fundamental distrust and paranoia. (There is quite a large chasm of paranoia and distrust between Kyle and Cartman). Quite similar to a Lovecraftian monster, his existence as a separate thinking entity is ultimately kept ambiguous.
Significantly creepy ending. Probably won't sleep for weeks. I'm surprised Trey and Matt went for something that dark and deep.
