Question about building a slave market
Does it continuously drain my reputation, or is it a one-time penalty?

It's constant, I am afraid. Destroy it.

I often conquer cities that already have it. When I first played, you did not even have the option to destroy it. It has been since added.

If playing VC Balance Mod, the honor penalty is disabled. The building’s positive effects didn’t seem valuable enough to warrant it, and it seemed a bit inconsistent with the rest of the game, which doesn’t penalize player honor for selling to slave traders or mines, serving as a slave hunter for villages, etc.

Does a slave market differ at all from a city like Ribe, where there is always a slave trader? I thought that the player was the only one who could build fief improvements.

The player is indeed the only one who can build fief improvements, but fiefs can be set to start the game with improvements already built, hence Tuidjy’s experience above with conquored fiefs.
