It also worth to mention that the Great Sword can be in any condition, so not only the plain Great Sword, but chipped or temperd ones are fine to fulfill the achievement.
as a female char, I gave the same Great Sword to 3 or 4 male characters, always the same first slot, and only managed to get the achievement at the 4th character.
So I sugest to try again until you get it.

I imagine most people find it confusing because when they hear "Lady of the Lake" and "a Great Sword", they automatically think of Excalibur. Since there are no "great swords" like Excalibur, even if they tried it with Nad and Draguandil, etc. it wouldn't work.
On top of that, you have the quirky specifics about which slot to put it into before you put it anywhere else.
So, if what you say is true, then you got lucky - good for you. Statistically speaking, a certain number of people had to have stumbled upon the solution accidentally.
This achievement is currently at 2.3% on global achievements...that makes it "elusive".
Your imaginings only apply to the smaller portion of the userbase that started out with VC.
For those that played native, it is very obvious what sword is being referred to as 'a great sword'. Although, I'm not certain it requires a great sword to trigger, it may well trigger off of any two handed sword. Can't exactly remove the achievement to test what triggers it.
I got it by giving a two handed sword to Ymira, iirc, specifically to gain that achievement.
Furthermore, the wording of the achievement does not place any specific emphasis on the great sword bit, meaning, that it isn't intended for people to assume it to be a named legendary weapon, especially obvious since it is a native achievement, which may well be why people are having issues obtaining it, since they are liable to be playing mods or VC, while the achievement is likely restricted to specific two-handed swords from native.
There are other achievements that reference Arthurian legend, and none of them are so specific as to attempt to match up with the actual context of the legend.
