Normally when your horse gets taken down in battle it has a chance to "degrade" by becoming "lame". I believe that once your horse is lame then it runs the risk of dying when it gets taken down.
Best advice I can give, if you're a mounted kinda guy, would be to keep a spare horse on hand at all times and to replace your mount if it becomes lame.
OR, (your horse fell dead...) message, and that can happen any time your mounts health reaches zero.

I had forgotten that there was both "lame" and "crippled". I was thinking of crippled and not lame when I initially posted my response.
I'm kind of surprised to see that people are having horses die outright fairly often. I wonder if its linked to your skills in some fashion. Personally, I always have high medical skills on my character and at least one of my companions.
To those who are having it happen a bunch... do you invest a lot in your medical skills?
If only the cavalry AI would do that. They always just seem to wedge themselves in the middle of the enemy. Maybe sending in infantry first would be a better idea?

Of course, that is just a guess. I've gone through quite a number of horses already. I even keep at least two in my inventory for swapping out/replacement.
I think I've found a horse in a drop maybe two times? That seems to be something that I would call very rare indeed.
Getting horses is a long-term thing for me. I buy them lame from the horse trader and heal them up myself. They are hard to find but if I keep my eyes open and make some effort to take care of my mount, it works out pretty well. Finding enough for all of your companions is a different story.
No, there is only Lame, not crippled.
the message ingame that shows up WHEN your horse becomes lame, is "Your horse was crippled" And the horse can die from any other status, lame or not, straight away, but the higher your surgery skill is, the most likely your horse is not struck into death.
When horse gets crippled, it becomes lame, as in "Champion Warhorse" becomes named "Lame Warhorse" which means its hurt, stats are reduced to really low (Speed goes down 15-10 or so, etc) and needs to be put in the inventory to heal. Wound Treatment skill makes healing horses IN INVENTORY faster, but they only heal into "warhorse" and not into "Champion warhorse".

I have not had a horse die on me in at least six months. It happend much more often whn I first started playing. I think the reason for this is that in the past I let my horses get taken down more often than I do now. I think it is just a random chance - perhaps something like a 10% chance to die instead of go lame or stay healthy. So, the more times you let your horse go down to zero health the more rolls you get for going lame or dying.
I take risks with my horse all the time circling the enemy and they do get injured all the time. But if they get hurt badly I will dismount and either swap horses with someone else, grab a random horse on the battle field, or just stay on foot.
There is a trick that I know of but rarely bother to use. A horse ridden by one of your troops or companions will never die or go lame but you go back to your own gear at the end of a battle so if you swap horses you can protect your horse. It is pretty gamey but works. Just use the dismount and mount commands to grab some other horse.
I also invest in lame horses in the markets. This works well to get some starter horses at a discount as long as you have somebody with medical skills. There is a chance for a lame horse to die after some time instead of heal but with medical skills you will very rarely see this happen if ever.
