And yes, anno domini 1257 is a mod. Though there are tons of historical mods out there.

P.S, you could try assassins creed 1 and 2, there in the middle ages.
Oh and almost forgot, try the guild 2 renaissance, you can't get much more realistic than that when it comes to games about the middle ages.
Some "fantasy" games might be accurate enough for your taste, even if there's some fantasy in them. For instance, the Prophecy of Pendor mod isn't a heavy "fantasy" mod. But, it has some fantasy elements in it that are very low-key additions. Besides, what we know of the Medieval period has been filtered by a rear-view mirror. In their world, there were witches, magic potions, elves, dragons, evil spirits and swords with magical properties. So, if you were transported back to the Medieval period, you might get yourself in a bit of trouble for denying what they beleived was "reality." :) (Prophecy of Pendor is an excellent mod, btw.)
Have you tried "The Guild" series? It's a psuedo medieval sim. I picked up "The Guild" pack ,recently on Steam, for pretty cheap. It came with The Guild, The Guild II and it's expansions. It's more of a late-medieval sort of period and it's focused on RPG, crafting and simulating the experience of a character in a medieval town.
AD1257 is often referenced when this subject comes up and it appears to be a very good quality mod for Warband. I haven't played it, but I trust the discriminating Warband player-base in this. There a quite a few mods that focus on Medieval Europe, of course. You may also like Gekoju, for a sort of "medieval" Japan version of Warband.
Stronghold: Crusader II is out on Steam. You might want to give it a try. (Stronghold III is out as well.)
The "Anno" series is extremely good. In the US pubslihed release, I play the version called "Dawn of Discovery:Venice." They're trade/city-builder sims and DoD:Venice is a lot of fun. Though, I recommend you get the community patch which fixes some of the bugs and a few issues with quests. (It's a great game and I think any fan of the Medieval period would like it.)
The Patrician series is supposed to be pretty good. I've looked over it and it does look interesting. Again, like many Medieval "sim" games, it's more of a trade dynasty sort of sim game. But, it has lots of medieval elements and atmosphere. Patrician IV is out, now.
