Yes, this game does do this to us. It is the single reason that despite many hundreds of hours and many "complete playthroughs", I have never actually finished this game.
Nothing works. The info on the Wikis is crap.
Here's what I've found. Sometimes a lord will "disappear" only to return again later on. I think this is them travelling outside of Calradia temporarily.
Sometimes a defeated faction will have lords on the map still so their faction isn't technically defeated until you chase them down. They could always take a small castle and start over from scratch, you see.
Sometimes, the game simply considers a trade caravan of a faction to count as a unit so you won't be informed of its demise until those traders get to their destination.
If there is a rebellion, defeated factions can sometimes come back.
Sometimes, the King of the faction (who can never die) will remain upon the map, refusing to give up and simply be a ghost that "haunts" little villages, etc. He'll move so quickly, you will likely never even see him and even holding all of Calradia and ruling over a peaceful and prosperous land will not be good enough to grant you victory.
I've had about three games get to that point and with no way to deal with ghost Kings, well...that is about as good as it gets.
There is no fix that I am aware of but I did hear that Ctrl+T (I think?) is supposed to highlight all the units on the world map...maybe that would work if you went out on your horse alone to find him?

For returning lords, make sure you are not running a mod that allows lords to return form exile or make sure the option is disabled at the end of a game.
If you are at the very end you can go ahead and use cheats to help but keep in mind that you will not get acheivements while they are enabled.
I'm sorry, was my contribution to this conversation some sort of sleight against you...in your mind?
Of course it is a known bug that has no solution, was there a comment in here that made you think someone needed to be told this?
Perhaps I'm just over-sensitive after spanking trolls on another thread but you seem to be getting unnecessarily snotty with me.
I'll still stand by every word I wrote because as you pointed out, it remains to be a problem with no known solution.
