When you finish your storymode campaign, you are given the option to continue in sandbox mode. If you choose to do so, it does nothing to your game other than unlocking the things you couldn't access before...like having a dog.
Getting married during storymode is kind of missing the whole point of being married and I'm not even sure it would be possible. Potential in-laws typically like their daughter's boyfriends to have wealth, renown and fiefs, etc. Eloping might work?
If you managed to find a wife in storymode, she'd stick with you when you transitioned into sandbox (assuming you didn't use cheat codes or something).
Romance is simply a part of the game. You can talk to people, some of them are interested in being more than friends. It's a lot of poetry reading and maybe fetching amber, etc. Then when you have enough renown, money and a castle, her daddy might give you his blessing.
Marriage isn't about having a pretty wife, it's about faction politics, relations with other lords, building allies, and stuff like that.
Once married, you can choose to have kids. You won't ever get to see them though. They don't really exist outside of the conversations you can have with your wife.
"How are my kids doing?"
"They're just fine"
That's about it.
