According to that guide, Ekkil indeed should die in the circumstances you described. But on the other hand, I found someone commenting under it that it didn't happen for them (as it doesn't happen for you). So it seems to me that Ekkil should die there and the game is set up for him to do that; but there is probably some bug that prevents that particular script from executing, that interaction never happens and Ekkil escapes his fate.
But yeah after I've done some digging of my own (Various playthroughs on youtube, reddit posts, etc) it seems no one has expirienced this end for Ekkil. So I guess there is a bug that prevented this scene from happening, similar to that of Krumr- Bolverk conversation that was supposed, but never happened in BS2. The only other alternative that pops to my head is that for some reason (either a bug or scripted) is that Ekkill doesnt die in this scene if Rook is the leader in the other caravan, as the guide you mentioned specifically mentions they tested with Alette, and myself and the youtube "lets play" I saw, had the same issue using Rook... So there's that, I will let you know if I find out something else, and thx again ...
