Tips for combat?

How do I get better at countering? It's hard to do when im about to finish a attack I can't switch to RT and Square at the same time quickly enough. Is it just a matter of reflexes?

Pretty much. When you're in a middle of attack/kill animation, hold RT to be prepared for incoming attacks and block them. And if you actually see someone attack you, go ahead and press X (while holding RT) before they hit you. Eventually you will learn how to time your attacks/counters to prolong your kill streak.
A few go-to tips:
1) Counters don't work on brutes (guards in heavy armor with big swords/axes) and seekers (spears). Dodge them instead and then attack.
2) You can only block up to 3 attacks, the third will break your guard and you'll take damage.
3) Practice your moves in virtual training. It helps a lot.

One more thing how do you throw sand in people's faces?

You have to hold the attack button while using the fists (D-Pad Down). The downside is that it takes time and leaves you completely open for incoming blows. Use it only against 1-3 enemies because otherwise you will constantly get hit while trying to collect sand.
