Killing a telepath frees the victims but beware, if the telepath is killed he explodes and any mind controlled humans that are near him may die from the explosion.
Which counts as having killed a human by the game.

ok thanks, I must have had that happen, i thought they died because they were linked to the telepath when it died

It does actually.

If you are going for the no human deaths achievment you just failed.

It also effects one the ending flags.

But i think that flag is bugged. I never killed any mind controlled human and rendered them all unconcious, either by killing the telepath or using the Stun Gun. Also got the acievement for never killing a human.
But at the very end the game still told me that most mindcontrolled humans died.

So basically it's the same flaw Dishonored 1 and 2 had. I remember my non-lethal run in Dishonored 2 when I reached a black market shop and the Shopkeeper died because of an explosion behind the house which I was not responsible for. Still the game said "it's your kill lil f***er"

Clearly not I could have sworn I kill some MC humans in my HA only run (also first run of the game) You can check my achs in my profile page.

The flag didn't seem bugged on my run. I didn't manage to save everyone because I didn't realize telepaths explode on death and 2 or 3 people died walking into a stream of fire coming from a pipe when I wasn't around (heard it though) but the game noted my attempt to save people in the end. I think I barely saved half the people I could've in Crew Quarters but did manage to save everyone else who could've died in other places.
