Start your Sim in Sunset Valley (I think that's where the Altos are). You've got a few options on this since there are two generations of Altos (Nick/Vita and Holly). You can use Master Controller to make your Sim a sibling of any of them, so that would make either Nick or Vita your sibling (and Holly your niece), or you could make Holly a sibling (making either Nick or Vita (or both) your parent(s)).
It gets a bit trickier if you want to be a cousin or something because you'll need to make a couple of extra households, set their relationships with the Altos, save them to the bin, start a new game, make your Sim, go into Edit Town mode, place the other households as NPCs, then use MC to set your Sim's relationship to those NPCs. Once that's done you should see the family tree filled in for your Sim.
I've used this method to link the Crumplebottom families in Sunset Valley (Agnes, Cornelia Goth, and their deceased parents) and Moonlight Falls (the three sisters). It was really tedious to make the family tree work for everyone, but it worked well once it was set up.
Click on target sims > NRaas > Master Controller > Advanced > Family > Add ??? > select designate sims
It's better to check their Family Tree and Relationships to ensure you done correctly too.
