Sounds like the insane trait.

- I've had a sim do similar except to their dogs pee puddle.

My Sim's borzoi was napping in his own pee puddle yesterday. Refused to get up so she could clean it. Guess when it's hot out you do what you gotta do.

a pee puddle is still a puddle; if not cleaned up a sim might play in it.

The real question is why is she peeing outside to begin with? If a sim needs to go they will prefer a toilet. So your sim must have had a full bladder and you refused to let her pee in a toilet.
