Some people "only" play normal mode and others play all modes, and that all so changes if you play with or without mods (like XP and drop rate mods to get things done faster or not).
If you play all content without any knowledge of the game and go slow and play each mode you can easily get hundred+ hours if you are good at ARPGs and know what you are doing for the most part and are good at building gear and looking up stuff on monster weaknesses and how to survive better in areas then you can get through the content much faster (and even faster with mods I talked about earlier if the game feels like a slog to you because drop rates are horrible because that's how it was back in the day and didn't have the BiS items drop like candy-like today's games).
That being said, that does not add in the new characters you can make, level-cap, skills, items, or additions that come with DLCs too, which can add more time investment.
Buying it right off the bat? Than NO Eternal Embers is not worth it. It only has the one difficulty setting which is the most difficult.
I'd say look at it after beating the base game, Immortal Throne, Atlantis and Ragnarok atleast on normal before thinking of buying.
Hard to say. There's quite a bit of content if you do everything. With AE + full expansions you'll have Act 1-4 from AE, Atlantis (Act 4.5), Act 5 (Rag) in each difficulty (beat final boss in Rag to unlock Epic then do same in Epic to unlock Legendary). With EE, beating final boss in legendary unlocks further campaign. With a twinked char with experience clearing Act 5 legendary can take ~40+ hours; on your first char and playthrough you can expect more time, maybe same amount to clear normal.
And this is for clearing with one char. The dual mastery system lets you make a ton of different class combos so you can potentially get tons of playtime by trying out different builds and playstyles.
For your reference, the speed run achievement for TQ is to complete legendary (minus EE) in 20 hours so even the speed run takes a while.
Eternal Embers adds new endgame content (Act 6+) exclusive to legendary difficulty. So you won't be able to access this new content until you finish the rest of the normal game.
It does add a new mastery (Neidan) which you can start immediately with but there's plenty of other masteries to try out so it's not vital to get until you finish the rest of the game and feel like you want to play more content.
