Body protection?
I wonder if helmets, vests are helping to protect the body and will decrease damage, or its only skin for visual?

I'm pretty positive it's just cosmetic.

It is, infact only a cosmetic item. Wearing any sort of helmet, vest, kneepadding or other usually protective gearing will not decline the damage your character takes.
