Click on it and look if something is missing. Another thing would be, if it is at the very edge of the colony, too far away from the general working places of your duplicants. They might simply pee on the floor before they reach it.
Assuming you are using outhouse, always make sure that your dups and fix everything, otherwise they won't empty them and you get pee all over. Be careful when you are prioritizing them in the beginning. Also readily available resources is always a bonus with the outhouse.
For the lavatory and any other thing that uses liquid piping(sink, shower, etc.), what matters is how your piping is set up. I'm dead serious. Check your piping. If you have it set up so that all of your pipes are in a straight line, chances are that the AI is going to assume the dups can't use the facilities because the pipe is blocked up all of a sudden, making the thing useless to the dups. It will continue until the AI finally realizes what's going on, and the dups might use the facilities.
Here's how to fix this. When laying out your liquid piping, make sure that the outflow of polluted water is raised from the outflow spot on the facility being used by at leas 2 squares. Yes, you use more pipes this way, but at least the pipes won't act like they are blocked up all the time.
