Can someone explain how can I further improve quality of my stuff on altars? Wiki says that you can combine basic items to get Super items, than combine super items to get ultra items or whatever. In my case dropping two super items as well as one common and one super did nothing.

If I remember correctly, you can only combine further with shadow weapons if you have the shadow relic turned on. Regular items do not combine more than once.

you can upgrade this way only shadow items (throw 4 swords to shadow altar, then merge in common one), there you can upgrade 4 times (24 swords or bows in total)

also there is dull ring, you can make super dull ring and then upgrade it with portion
or upgrade dull ring with same portion 2 times and then merge into super dull ring

common items can be upgraded only 1 time to super-* item
