Focus is the best Rei talent
Rei is difficult to understand because of all the individual small effects of her kit. She is maybe the most divisive champ when it comes to choosing a talent. Here is why focus is the best.

Normal lifelink ult generation over 7 seconds: 3.5%

Focus lifelink ult generation over 7 seconds: 6%

Normal lifelink healing over 7 seconds: 350

Focus lifelink healing over 7 seconds: 600

The reason I chose 7 seconds is because 7 seconds is the no chronos cooldown time for Focus lifelink, these values come from the 4 active seconds of Focus and the 3 seconds of cooldown.

When you factor in chronos, focus takes an even further lead in healing and ult generation, and eventually only has an extremely short inactive time (0.9s). This improves scaling throughout the game, which is something people often overlook. Supposedly focus also slows enemies by 30% instead of 10% when linked as well.

The biggest drawback to focus is ultimate flexibility, and not being able to give the chain heal spirit link healing bonus (30%) to targets as often. It does require more thoughtful planning in the early game, but becomes much less noticeable further into the game.

It's also interesting because you are stronger in 1v1 situations and you can dps more, but, the problem is that it depends against what comp you play and with who you play. Spiritlink is mostly used against enemies and then it goes on CD... so you can't really switch to another target to protect your teammates (if you want to switch and ult someone or give dmg reduction or give additional heals for example). In a double sup comp, Focus is the best talent hands down. But, solo sup Rei has a lot of flaws because your teammates will always lack heals, but not only that, when your allies take damage and you try to do damage to get your CD back, your other dps will just wait behind something for idk how many seconds without doing any damage... and sometimes you can't dps so the CD becomes more problematic too. Its never fun when you can't dps for a long time because you have to wait for the auto-heal or wait for Rei's heal. And like I said earlier you will most likely use your spiritlink to dps so you can't really use it to save one of your allies when they are in danger. I think that Focus Rei works very well when you play an aggressive comp and that your teammates understand that they need to stick with Rei, otherwise, her heals feel lacking 'cause a lot of players do not understand that they need to stay around Rei to protect her so she can support her allies, dps, heal and ult. And if your teammates can't understand that Rei does not have infinite range then better use the other talent.

I pick focus when i want to load ult into others.

i pick first talent if i want to heal the whole team. deft hands and aico forces makes her cooldowns go extremely faster, most of the time i've outclassed all healers in my games. i'm level 140 rei player and believe it or not if you abuse aico forces (linking) with macro you will give crazy heals.

Looking on other talents of Rei, this one is most impactful, both offensively and as support tool.
Caute LC is situational as hell, and useless mid-late game, and forces You to use 2 abilities on one enemy at once, bad trade in my opinion... Range LC in theory sounds fine, but its just range, I rather stay close to team, same final result.

Extension (range talent) is supposedly bugged/situational and sometimes results in reduced healing overall. I have found it to be useful when you have a split team on flat maps like shattered desert.

Restraint (Anti-heal) is definitely pretty strong if you don't like the drawbacks of Focus. Catching a tank like fernando/khan when they put up their shield and preventing them from being healed is a huge bonus.
