The other DLCs are mostly visual content.
However if there is a sale for them, there isnt much harm in buying them all.
If not, riddle of steel is the second good one, and people of the East the third imo.
None of them are required to have a great time with the game.
Other than that, just pick the one/s in which you enjoy the look of the building set included (if included).
NOTES:
Unless absolutely needed, the RIDDLE OF STEEL pack can be avoided unless you are a completionist as it just contains statues and other placeables based on the Arnie Conan the Barbarian movie.
The DEBAUCHERIES OF DERKETO pack is mostly filled with content geared towards RP - outfits, tavern placeable, jewelry etc. The small amount of building pieces included are situational.
What else Siptah adds is materials and placeables that have the same function the original game ones have.
It is not the you have any "expanded" game features on Siptah.
If nothing else, Siptah is even a cheesier game level than Exiled Lands is.
Debaucheries of Derketo for Bejeweled Set and lotsa great and sexy outfits/armor, emotes, furniture for RP.
best to buy after geting it finished.
