With the recent attribute rework they are arguably just better looking. To OP, buy the base game and try it out. If you enjoy it, you will have more knowledge to decide if and which DLCs you want the most.
If you are playing PVE then all the DLC's are cosmetic only. Only if you are a hardcore PVP player who likes to minmax everything you NEED to have certain DLC's.
If you are a type of player who builds a grey cube for his base then you don't need any DLC's. If you are into building castles in different styles and like to have lot's of decorations and placeables, then you might want to look into getting some of DLC's.
On single player game or on your own server you can also want to look some mods that give you additional building sets and placeables.
I would agree with Narelle, play the base game and then decide if you want any DLC's, perhaps wait for a sale.

Practically none. Almost all DLC is cosmetic only.
Isle of Siptah is the only DLC that adds content: a whole different map with lots of different content. Many have however claimed that Siptah is somewhat low on content and boring compared to the exiled lands (which you get as part of the base game)
However you can only have one character on one map per server. So that one character cannot swap maps. You would have to choose which map to play, and I wholly recommend the Exiled lands for a starting player. Exiled lands comes with the basic version of the game.
ps. I also recommend that you wait for Steam discounts before buying DLC.
But you should definitively get Isle of Siptah first cause that is a whole new map with lots of new stuff.
I would say something in this order:
1. Siptah map
2. Turan
3. Derketo
4. New frontier, picts (this one and Turan fits in the most aesthetically)
5. People of the dragon if you like castles or churches in sort of medieval style.
Whatever you want after those.
