Wearable Witch Queen Mask?

Anyone figure out how to get the equipment version of the mask? Aside from admin panels.

I always get both when I kill her, but I play SP. It actually screwed me over as I thought they were both the same, and as such grabbed the wearable one (never intending to wear it) thinking I just needed one to complete the game. When I got to the altar at the end, and tried to craft the Keystone, nothing happened even though I had everything (or so I thought).

In short, if anybody is having trouble with the Chaosmouth Altar not working, it might be that you have the wrong Witch Queen's mask.
