Which one is worth it? should i use it or put in on my thrall?

Both if possible. In general it is better for you to do it. The effects stack (20 stacks of bleed, 10 stacks of poison), and you are far more likely to get the attacks off quick enough to stack them up and keep them going.

could i ask if the dmg scales with hp?

Its a fixed amount per stack. For example, Bleed deals 1.35 damage per second per stack (assuming the wiki is still correct), and maxes out at 27 damage per second at 20 stacks. It will last for 8 seconds after the last attack that causes Bleed hits. Hitting again while the duration is still going on will add more stacks (if possible) and reset the duration.
